I provide insurance advice and services through iAssure Inc.. These insurance activities are conducted as an outside business activity (OBA) and are neither the business nor the responsibility of WhiteHaven Securities Inc.

I distribute mutual funds exclusively through WhiteHaven Securities Inc., a registered mutual fund dealer. I do not speak for, nor do I represent, WhiteHaven Securities Inc. beyond my capacity as a Mutual Fund Dealing Representative.

2. Two privacy regimes apply

Because I operate in two regulated sectors, your personal information may be handled under different privacy frameworks depending on the service:

I, through iAssure Inc., am responsible for collecting, using, and safeguarding personal information related to life, disability, critical illness, and group insurance products.

Mutual Fund Activities and WhiteHaven Securities Inc. (Privacy)

Mutual fund accounts and related personal information fall under the custody, control, and responsibility of WhiteHaven Securities Inc.

iAssure Inc. does not administer or control personal information related to mutual fund accounts.

3. Personal information collected (iAssure Inc. — insurance)

For insurance advice and product placement, I collect personal information that may include:

• Contact information
• Date of birth and identification details
• Financial information relevant to insurance needs
• Health and lifestyle information required for underwriting
• Business ownership or employment information
• Beneficiary designations
• Records of communications, meetings, and signed documents

This information is required to:

• Analyze insurance needs

• Prepare applications and submit them to insurers
• Assist with underwriting requests
• Service existing policies
• Meet legal, regulatory, and recordkeeping obligations

4. How information is collected

I collect personal information directly from you when you:

• Sign forms electronically or in person

I may also receive information from:

• Insurance carriers (underwriting, policy details, medical questioning)
• Medical professionals (with written consent)
• MGAs and service providers assisting in processing applications

5. How information is used (iAssure Inc.)

• To prepare, submit, and administer insurance applications
• To assist during medical underwriting
• To service policies over time
• To comply with legal and regulatory recordkeeping requirements
• To communicate administrative and service-related updates

I do not sell or rent personal information.

6. Disclosure of information (iAssure Inc.)

Your information may be shared with:

• Insurance carriers underwriting or servicing policies
• MGAs providing administrative support
• Medical providers involved in underwriting (with your consent)
• Technology providers (CRM, secure cloud storage, e-signature platforms)
• Regulators, if required by law
• Your accountant, lawyer, or notary — only with your explicit consent

Activities conducted through iAssure Inc. are neither the business nor the responsibility of WhiteHaven Securities Inc.

7. Cross-border storage and hosting

Some service providers may store or process information outside Québec or Canada, where local laws may permit access by authorities. Providers used must maintain protections consistent with applicable privacy laws and contractual obligations.

By engaging my services, completing forms, or submitting applications, you consent to the collection, use, and disclosure of your personal information as outlined in this Notice.
